Job Summary

We are seeking a skilled Medical Geriatric Specialist to join our team at MedCentric in Stockton, CA.

The estimated salary for this position is $110,000 - $140,000 per year, depending on experience.

About the Role

This role involves collaborating with Primary Care Physicians, Community-Based Primary Care Physicians, and an interdisciplinary team (IDT) to deliver comprehensive care for our participants.

  • You will conduct primary and acute care evaluations for participants in collaboration with other healthcare providers.
  • You will perform comprehensive in-person assessments, both scheduled and unscheduled, and collaborate with the IDT to develop personalized care plans.
Responsibilities

Your key responsibilities will include:

  • Conducting primary and acute care evaluations for participants in collaboration with other healthcare providers.
  • Performing comprehensive in-person assessments, both scheduled and unscheduled, and collaborating with the IDT to develop personalized care plans.
  • Managing discharge planning for participants leaving acute or long-term care settings.
  • Coordinating participant care in nursing homes, providing regular visits and overseeing ongoing treatment.
  • Being an active member of the IDT, working with physicians and other care team members to ensure seamless care and 24-hour delivery when needed.
  • Participating in rotating night and weekend call to support participant needs.
  • Ensuring timely and thorough documentation to support participant care and team collaboration.
Requirements

To be successful in this role, you will need:

  • A graduate degree from an accredited School of Nursing with RN State License.
  • Certification as a Nurse Practitioner with Prescriptive Authority.
  • Active DEA Registration and eligibility for Medicare and Medi-Cal enrollment.
  • Reliable transportation for community-based care.
