We are seeking a highly skilled Registered Nurse to join our team as a Community Health Liaison, Home Care Specialist. This dynamic position is responsible for accepting referrals for Hospice, Home Health, and Home Infusion services in various settings, including acute care, physician clinics, and nursing homes.
Key Responsibilities- Establish and maintain relationships with discharge planners, medical social workers, physicians, and other healthcare professionals to facilitate timely discharges and support continuity of care.
- Deliver excellent customer service and provide education on our services to patients, families, and healthcare providers.
- Assist in the facilitation of referrals and ensure seamless transitions of care.
- Collaborate with the healthcare team to identify opportunities for improvement and implement changes to enhance patient outcomes.
- Stay up-to-date on industry trends, best practices, and regulatory requirements to ensure compliance and quality care.
- Current RN license in Missouri
- Minimum three years of nursing experience, with a focus on Hospice and Home Health
- Excellent communication, observation, and assessment skills
- Ability to work independently and as part of a team
- Basic computer skills and understanding of Electronic Medical Records
