Key Responsibilities:

The Clinical Nurse Case Manager will be accountable for all aspects of Clinical Care Coordination and Case Management for a designated caseload, which includes patients with diverse acute and chronic health needs. This role involves:

  • Conducting new patient consultations and admissions
  • Creating comprehensive and individualized care plans
  • Performing ongoing physical and psychological assessments
  • Managing medications and conducting in-home evaluations
  • Facilitating professional communication among the medical team, patients, and families to ensure continuity of care
  • Providing education to patients, families, and caregivers
  • Making referrals based on patient requirements
  • Supporting patients during office visits, including necessary documentation
  • Maintaining and reviewing electronic medical records
  • Participating in primary care conferences

Qualifications:

Applicants must possess the following qualifications:

  • Current Registered Nurse license in California
  • Graduation from an accredited nursing program
  • Preferred 5 years of nursing experience, including acute care; however, strong candidates with less experience will be considered
  • Excellent patient assessment and organizational skills
  • Current BLS certification
  • Exceptional written and verbal communication abilities
  • Capability to represent patients professionally and tactfully
  • Ability to work independently and efficiently
  • Proficiency in using technology, including iPhone and MS Office 365
  • Familiarity with Clinical Based EHR/EMR Systems
  • Valid California driver's license and reliable transportation
  • Proof of personal RN liability insurance
  • Experience in Case Management, Home Health, Hospice, or Community Nursing is a plus
  • Certification in Case Management is advantageous
