Qualifications:
  • Active Occupational Therapy license in Arizona.
  • Experience in home health settings.
  • Adherence to professional standards and practices.
  • Strong verbal and written communication skills along with excellent interpersonal abilities.
  • Knowledge of federal and state regulations related to home care.
  • Valid driver's license with automobile insurance as per agency and state requirements.
  • Current CPR certification.
  • Recent TB test results.

Core Duties:
  • Implement individualized Plans of Care.
  • Deliver skilled interventions aimed at achieving realistic client outcomes within designated timeframes.
  • Communicate significant changes in client status to the RN, physician, and other team members promptly.
  • Exhibit proficiency in therapy skills in accordance with personal and legal scopes of practice.
  • Provide therapy with respect for individual, cultural, and spiritual differences.
  • Maintain accurate client records reflecting interventions and education.
  • Ensure clinical documentation is accurate, complete, and timely, in line with agency policies, regulatory requirements, and industry standards.
  • Demonstrate knowledge of safety and infection control practices by adhering to policies and regulations.
  • Promote a safe environment for clients and colleagues.

Essential Functions:
  • Conduct occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluations for patients under a medically approved Plan of Care.
  • Complete assessments promptly and in accordance with agency policy.
  • Develop treatment programs and set goals for enhanced functionality.
  • Provide assessment findings and goals to aid in the development of patient care plans and obtain specific physician orders as necessary.
  • Document all findings, plans, interventions, and patient progress (outcomes) on designated forms and submit as per agency guidelines.
  • Communicate plans and changes to the physician and nursing Case Manager, as well as other agency staff through care plans, written progress notes, and participation in care conferences.
  • Guide and instruct patients in therapeutic self-care and creative activities aimed at improving independence and physical and cognitive functioning.
  • Advise, consult, and educate patients, family members, and other agency staff on appropriate treatment procedures and supportive activities.
  • Design and/or order suitable adaptive/assistive devices and/or splints to enhance optimal functioning.
  • Instruct patients and their families on the specific patient program.
  • Perform duties in compliance with agency policies and procedures as well as professional and community standards.
  • Provide services in accordance with applicable state, federal, and JCAHO standards.
  • Participate in meetings and required educational programs.
  • Take responsibility for personal and professional development and identify learning needs to supervisor.
  • Attend care conferences, initiate interdisciplinary referrals, and engage in care coordination as needed.
  • Serve on the Clinical Record Review Committee.
  • Carry out other related duties and responsibilities as necessary.

Supervision of Occupational Therapy Assistants:
  • Instruct and supervise Occupational Therapy Assistants in specific phases of occupational therapy services they may provide to patients.
  • Be available by phone to Occupational Therapy Assistants at all times during patient treatment.
  • Supervise on-site while Occupational Therapy Assistants provide patient care—every four (4) to six (6) visits, minimally once a month. Supervision does not equate to treatment.
  • Conduct on-site functional assessments, review and revise the Plan of Care, and assess the utilization of community resources.
