As a Home Infusion Pharmacist at AmeriPharma, you will be responsible for providing pharmaceutical care to patients who receive intravenous medications or therapies in their homes. This role requires strong knowledge of aseptic technique, compounding practices, and home infusion therapy equipment and procedures.

Responsibilities:

  • Conduct thorough assessments of patients to determine their suitability for home infusion therapy
  • Collaborate with the nursing department to ensure a home health nurse is employed prior to the start of care, if appropriate
  • Work with healthcare providers to coordinate the transition of patients from hospital or clinic-based care to home infusion therapy
  • Verify patient insurance coverage and obtain necessary approvals or explore alternative options if coverage is not available
  • Review prescription orders for accuracy, completeness, and appropriateness
  • Communicate with healthcare professionals to clarify prescription details or obtain additional information
  • Recommend a TPN formula based on clinical experience and practice guidelines
  • Maintain and update patient profiles in the pharmacy's system
  • Oversee inventory management, including ordering and maintaining an adequate supply of infusion medications and related supplies
  • Ensure accurate compounding and dispensing of pharmaceuticals according to company protocols and physician orders
  • Determine appropriate selection of infusion supplies
  • Educate patients and their caregivers on proper medication administration, storage, and disposal
  • Respond to emergency situations, such as adverse drug reactions, line infections, and equipment malfunctions

Requirements:

  • Unrestricted active CA Pharmacist license
  • Strong knowledge of aseptic technique and compounding practices
  • Knowledge of home infusion therapy equipment and procedures
  • Excellent verbal and written communication skills
  • Excellent interpersonal and customer service skills
  • Ability to prioritize tasks and delegate when necessary
  • Ability to function well in a high-paced and at times stressful environment
  • Proficient with Microsoft Office Suite or related software

Compensation:

The estimated salary range for this position is $72-$74 hourly/DOE, depending on experience and qualifications. A comprehensive benefits package, including medical, dental, vision, life insurance, and 401k plan, is also offered.
