Job SummaryWe are seeking an experienced Nurse Practitioner or Physician Assistant to join our team as an Advanced Chronic Disease Care Specialist. This role will be based remotely within the state of employment and requires occasional travel for training and meetings outside of the state.
Key Responsibilities- Conduct comprehensive annual wellness exams on patients both in their homes and in virtual environments.
- Counsel and educate patients and families about benefits and programs available to help them live healthier lives.
- Document patient information, including chief complaints, diagnoses, medical history, family and social history, review of systems, examinations, medications, allergies, assessment, and plan.
- Coordinate care with primary care providers, specialists, and ancillary services.
- Complete all documentation and paperwork in a timely manner.
- Maintain quality of care standards defined by the practice.
To be successful in this role, you will need:
- A Registered Nurse and Nurse Practitioner or Physician Assistant license.
- Board certification for your licensure (NP: ANCC/AANP; PA: NCCPA).
- A current and unrestricted DEA certificate.
- The ability to work autonomously and without direct supervision.
- Proficiency with medical instruments and equipment required for the job.
- Knowledge of computer-based data management programs and medical records/point-of-interview technology.
- Effective verbal and written communication skills.
- A sound understanding of federal and state regulations, including HIPAA and OSHA.
- At least 2 years of direct patient care experience.
- Managed Care/IPA/Health Plan experience.
- Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.
