Job Summary

We're seeking a compassionate and skilled LPN to join our hospice team as an On Call Evening/Night Shift caregiver. As a member of our team, you'll provide direct patient care, implement current nursing practices, and participate in the coordination of hospice services.

Responsibilities
  • Direct Patient Care: Provide care as defined in the State Nurse Practice Act, ensuring the highest level of patient comfort and well-being.
  • Implement Current Nursing Practices: Follow a comprehensive assessment and plan of care to deliver effective patient care.
  • Documentation: Accurately and timely document patient services to reflect the plan of care.
  • Patient Education: Assess and provide education to patients and their families/caregivers on diagnosis and self-care plans.
  • Interdisciplinary Coordination: Participate in the coordination of hospice services, reporting identified needs to the interdisciplinary group.
  • Pain/Symptom Management: Provide appropriate pain and symptom management, evaluating patient responses to treatments/medications.
Requirements
  • Nursing Experience: Three years of nursing experience, with community health/hospice or medical/surgical experience preferred.
  • Licensure: Currently licensed as an LPN/LVN in the practicing state.
  • CPR Certification: Maintain a current CPR certification.
  • Driver's License: Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
