Job Summary

The Nurse/Health Educator, Specialized Preventive Services, plays a vital role in ensuring the health and well-being of children in our care. This position requires a skilled and compassionate professional who can provide medical support, education, and guidance to families and caregivers.

Key Responsibilities
  • Conduct Intake Assessments: Conduct thorough assessments of clients and gather essential medical history information to inform case planning and service delivery.
  • Monitor Child Health: Regularly monitor the health and well-being of children, assess safety and risk, and provide education and support to families and caregivers.
  • Provide Health Education: Educate parents and caregivers on essential health topics, including immunization, medical exams, nutrition, and disease prevention.
  • Coordinate Medical Services: Arrange and accompany children and families to medical appointments, ensure follow-up care, and obtain specialist reports as needed.
  • Support Families: Provide emotional support and guidance to families and caregivers, addressing concerns and providing resources to promote healthy development.
  • Maintain Accurate Records: Ensure accurate and up-to-date records of client information, medical history, and service delivery.
  • Participate in Staff Meetings: Collaborate with the team to share knowledge, best practices, and concerns to ensure high-quality service delivery.
Qualifications
  • Education and Licensure: Associate or Bachelor's degree in nursing and licensure to practice in NYC.
  • Language Skills: Bilingual/Spanish and experience working with medically fragile populations are highly desirable.
  • Communication Skills: Excellent written and verbal communication skills to effectively provide direction, counseling, and education to clients and families.
  • Time Management: Ability to prioritize tasks, manage time effectively, and work independently with minimal supervision.
