Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team. The successful candidate will assist the Registered Nurse (RN) in patient care, completing assigned tasks in accordance with the New York State LPN scope of practice. This role requires strong communication skills, a customer service orientation, and a problem-solving approach to ensure excellent patient outcomes.

Key Responsibilities:
  • Collect patient healthcare data systematically and on an ongoing basis using appropriate assessment techniques and instruments.
  • Administer medications and vaccinations, including injections, under the direction of the provider.
  • Provide telephonic support for patients, scheduling problem visits or nurse visits when appropriate, and offering symptom management under the direction of the provider.
  • Monitor and coordinate delivery of the care plan, forming relationships with patients and their caregivers to support prevention-focused care and ED/hospital diversion where appropriate.
  • Conduct medication and testing authorizations, and deliver an exceptional patient experience through service, responsiveness, and care.
Requirements:
  • Current state license to practice as an LPN.
  • CPR Certification required.
  • EMR experience.
  • Strong communication skills and customer service orientation.
  • A problem-solving orientation, accountability, and a flexible and positive attitude.
  • A supportive attitude toward our patient population.
  • 2 or more years of healthcare experience working in a primary care office or other outpatient setting.
  • US work authorization.
