Job Title: Diabetes Education Specialist

M Fairview Health Services has an immediate opening for a Diabetes Education Specialist (RD) based out of Southdale Hospital located in Edina, MN.

About the Role:

This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible position. As a Diabetes Education Specialist, you will be responsible for providing education and support to patients with diabetes, as well as their families and caregivers.

Responsibilities:
  • Develop and implement individualized education plans for patients with diabetes
  • Provide education on diabetes management, including medication management, nutrition, and physical activity
  • Collaborate with healthcare teams to ensure comprehensive care for patients with diabetes
  • Conduct group and individual education sessions
  • Assess patient knowledge and understanding of diabetes management
Requirements:

To be successful in this role, you will need to have a strong background in diabetes education and management, as well as excellent communication and interpersonal skills.
