Job Summary

We are seeking a skilled and compassionate Home Care Field RN to join our team at Carle Health. As a member of our interdisciplinary team, you will provide direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental, and spiritual needs of our patients and families.

Responsibilities
  • Coordinate and provide direct patient care using the nursing process
  • Assess, plan, implement, and evaluate patient care
  • Interact with clients, caregivers, and families to assess, plan care, arrange services, monitor, and provide support and education
  • Complete admission documentation within policy guidelines
  • Ensure physician orders are written timely and corrections to care plans are entered and transmitted according to timeline
  • Review verbal orders with MD office and ensure proper acronym is evident on all physician orders
  • Collaborate with other disciplines, MD, care coordinator, insurers, and other healthcare professionals to maintain proper linkages within the continuum of care
Requirements
  • Associate's Degree in Nursing or Diploma in Nursing
  • Registered Professional Nurse (RN) License in Illinois
  • One year of healthcare experience
  • Ability to work as a member of an interdisciplinary team
  • Current CPR certification
