Job Title: Clinical Care Transition Educator

At Option Care Health, we are seeking a highly skilled Clinical Care Transition Educator to join our team. As a Clinical Care Transition Educator, you will play a critical role in ensuring the successful transition of patients to our care delivery model.

Job Summary

The Clinical Care Transition Educator will be responsible for providing clinical education and training to patients, caregivers, and facility staff. This includes evaluating patient viability, conducting patient assessments, and developing educational materials to support successful transition.

Key Responsibilities
  • Evaluate patient viability and develop individualized education plans to support successful transition to our care delivery model.
  • Conduct patient assessments and evaluations to determine patient suitability for our care delivery model.
  • Develop and deliver educational materials to patients, caregivers, and facility staff to support successful transition.
  • Partner with the sales team to understand clinical transition educational needs and develop improvements to increase patient transition volume.
  • Maintain confidentiality of patient and proprietary information and observe legal guidelines for safeguarding confidentiality.
Requirements
  • Active and unrestricted Registered Nurse (RN) license required.
  • Minimum of 2 years of experience in the healthcare industry.
  • Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors.
  • Experience applying knowledge of standard practices for all services offered as well as current relevant and applicable standards.
  • Basic PC skills: Able to competently use internet, email, Microsoft Word, Microsoft Excel, Microsoft PowerPoint.
  • Experience in identifying operational issues and recommending and implementing strategies to resolve and improve processes.
  • Access to a reliable means of transportation which will enable the incumbents to travel to care facilities, home visits and multiple hospitals.
Preferred Qualifications
  • Hands on home or alternate site infusion or discharge planning experience.
  • Experience growing service provider partnerships.
