Job Summary / Purpose
The Case Manager RN is responsible for coordinating a comprehensive system of case management to ensure patients receive continuous care throughout their healthcare journey within a diverse healthcare delivery system.
This role involves assessing patient conditions, evaluating ongoing needs, planning care delivery, and implementing community resources through effective discharge planning. Additionally, the Case Manager RN will monitor the quality of services throughout the case management process and perform direct utilization reviews on all acute admissions.
Essential Key Job Responsibilities:
- Adheres to safety and emergency protocols.
- Follows Hospital/Department/Unit policies and procedures.
- Documents activities in accordance with Hospital/Department/Unit guidelines.
- Executes critical and frequent procedures or services.
- Provides care and services based on identified patient needs.
- Utilizes necessary equipment for delivering care and services.
- Consults with Hospital/department resources as needed.
- Exhibits professional responsibility as per job classification.
- Demonstrates Core Values in Guest Relations and Service Standards.
- Upholds Core Values based on Behavioral Standards.
- Performs additional duties as assigned by management.
Qualifications:
Minimum Qualifications
Required Education:
Graduate from an accredited nursing program.
Required Licensure and Certifications:
Kentucky state licensure as a Registered Nurse.
Required Minimum Knowledge, Skills, and Abilities:
Preferred candidates will have at least 2 years of recent experience in an acute hospital setting.
A strong clinical background with diverse clinical experience is essential, along with the ability to utilize data and regulatory information in decision-making processes.
Proficiency in conversational English is necessary for effective communication with management, patients, and colleagues, as well as for understanding safety protocols and hospital policies.
PREFERRED Qualifications:
Education:
Associates Degree in Nursing, CCM, Interqual, M&R, or Discharge Planning Certification.
Experience:
3-5 years of recent experience in an acute hospital environment, with at least 1 year in Case Management or home health, particularly with Medicare/Medicaid guidelines, or 2 years in Utilization Management within a hospital setting. Experience in discharge planning using a case management model is also desirable.
