Join a prestigious academic health-care institution as an Advanced Heart Failure Nurse Specialist within the Advanced Heart Failure/Pulmonary Hypertension program.
The Advanced Heart Failure Nurse Specialist is responsible for overseeing patient care and assessing the appropriateness and outcomes of treatment in collaboration with the patient care team.
The Advanced Heart Failure Nurse Specialist is dedicated to achieving optimal patient care outcomes throughout the continuum of care by adhering to the established standards and protocols set forth by the University of Chicago Medicine and the State of Illinois.
Key Responsibilities:- Develops, monitors, and coordinates patient care plans utilizing clinical pathways.
- Facilitates care transitions between inpatient and outpatient environments.
- Engages with patients, families, and multidisciplinary teams to ensure safe, effective, and timely care.
- Identifies and addresses patient and family needs for specialist consultations.
- Ensures educational needs of patients and families are met.
- Demonstrates expertise in cardiovascular and advanced heart failure patient management.
- Works autonomously while collaborating with healthcare providers on a daily basis.
- Conducts patient rounds in the hospital for education and assessment purposes.
- Collaborates with specialty pharmacies for the management of treatment regimens.
- Provides on-call support for inquiries and medication adjustments.
- Educates nursing staff on the care of patients with pulmonary hypertension.
- Participates in clinical settings alongside physicians.
- Manages communication for pulmonary hypertension and advanced heart failure patients.
- Current licensure as a Registered Nurse in the State of Illinois.
- Current Basic Cardiac Life Support (BCLS) certification.
- A minimum of two years of experience in advanced heart failure management.

- Shift: Days, 7am-5pm; 10-hour shifts
