About the Role:

Duties and Responsibilities:

* Participate in quality improvement initiatives to enhance patient care and safety.

Requirements:
* Current licensure as a Registered Nurse in the State of California.
* BS or higher degree in Nursing preferred.
* Experience working in ambulatory care settings, with a focus on transitional care management.
* Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
