Position Summary: The selected candidate will be responsible for providing comprehensive care to adult patients, ensuring a seamless continuum of care.
Key Responsibilities:
- Conduct thorough examinations and provide treatment to maintain continuity of patient care.
- Perform detailed assessments of patients, including medical history, presenting issues, allergies, psychosocial factors, medications, and living conditions.
- Order, interpret, and evaluate diagnostic tests to accurately identify and address patients' clinical concerns and healthcare requirements.
- Recognize the need for patient and family education and make appropriate referrals.
- Engage in a collaborative, multidisciplinary approach focused on the needs and care of clients.
- Consult with various healthcare professionals, including case managers and family planning specialists, involving family members in care planning as necessary.
- Execute physical examinations and preventive health measures in accordance with established protocols and physician guidelines.
- Document physical findings and develop a prognosis based on the patient's condition.
- Assume additional responsibilities as required to support the mission of the organization.
Qualifications:
- Active Nurse Practitioner License in Massachusetts
- Master's Degree in Nursing
- Board Certification in the relevant specialty
- Current CPR Certification
