Job Title: LMSW/LMHC Client Care Coordinator

Job Summary:

We are seeking a highly skilled and compassionate LMSW/LMHC Client Care Coordinator to join our team at Women In Need, Inc. The successful candidate will be responsible for providing clinical services to individuals and families living in our transitional housing sites.

Key Responsibilities:

  • Conduct psychosocial assessments to identify the needs of individuals and families.
  • Engage clients in short-term counseling, groups, and workshops to promote long-term services.
  • Refer clients to other service providers within the community for comprehensive support.
  • Participate in Win meetings to address the needs of adults and children receiving clinical services.
  • Collaborate with Win colleagues and outside agencies to ensure consistency in service delivery.
  • Provide crisis intervention to clients and identify those who require a higher level of service.
  • Develop and implement individual service plans for children and engage parents in the process.
  • Conduct play groups for children to address issues of trauma and facilitate workshops and groups as needed.
  • Collaborate with DOE, medical, and mental health providers to ensure the needs of Win children are addressed.

Requirements:

  • LMSW/LMHC degree required, with the ability to obtain within the first 90 days of employment.
  • Experience facilitating groups and developing group curricula.
  • Experience working with individuals with mental illness and substance abuse, preferably with homeless adults and at-risk populations.
  • Advanced knowledge in the field of social work/mental health.
  • Ability to work evenings, at least one Saturday a month, and some holidays.
  • Must be available for on-call rotation and attend required internal and external meetings.
