Job Summary
We are seeking a highly skilled Social Worker/Therapist to join our team at The Family Care Network Inc. As a Behavioral Health Specialist, you will work closely with clients, families, and other professionals to deliver high-quality services and support. Your expertise in assessment, care planning, and therapeutic interventions will be essential in helping clients achieve their goals and improve their overall well-being.

Essential Duties and Responsibilities
Conduct comprehensive assessments to evaluate client mental health, emotional well-being, and social functioning. Develop individualized care plans in collaboration with clients to address their specific needs and goals. Provide direct behavioral support, evidence-based interventions, and life skill development to support clients with achieving their goals. Engage clients in meaningful activities and social interactions to promote mental and emotional well-being, including providing transportation and/or accompanying clients to access community resources and support services. Maintain accurate and timely clinical documentation in accordance with agency timelines, policies, and confidentiality standards. Ensure documents meet Medi-Cal and other regulatory requirements.
