Salary: The estimated salary for this role is $85,000 - $110,000 per year, based on national averages and industry standards.

Job Description: As an RN Hospice Care Manager at LHC Group, you will be responsible for delivering high-quality patient care and developing trusting relationships with patients and their families. Your primary focus will be on evaluating patient needs and re-assessing them as necessary, while collaborating with the patient's attending physician and other members of the IDG to ensure comprehensive care.

Responsibilities:

  • Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral
  • Identify patient physical, psychosocial, and environmental needs, and re-assess them as needed
  • Initiate and coordinate the plan of care, maintaining accurate and relevant clinical notes regarding the patient's condition
  • Collaborate with patients, families, and healthcare professionals to deliver high-quality patient care

Required Skills and Qualifications: To succeed in this role, you will need:

  • One year of clinical experience as a registered nurse
  • Current RN licensure in the state of practice
  • CPR certification
  • A valid driver's license and access to a dependable vehicle or public transportation
