About Your WorkThe Care Manager (LVN) is a critical role that involves partnering with external providers across the continuum of care to enhance the overall quality of care delivered to our members. As a Care Manager, you'll be responsible for identifying, developing, coordinating, promoting, supporting, and implementing assigned clinical quality and/or service improvement projects and programs. You'll act as a resource for evidence-based practice integration, provider training/education, policy review/development, and regulatory/accreditation interpretation. You'll manage multiple complex initiatives that impact the quality or effectiveness of healthcare delivery and/or healthcare services, working in conjunction with both external facing and plan quality improvement leaders and key stakeholders to provide support for multi-disciplinary teams.
Your Key Responsibilities- Conduct initial assessments of patients' health status and needs.
- Develop individualized care plans in collaboration with the healthcare team.
- Coordinate and schedule patient appointments, treatments, and procedures.
- Monitor and evaluate patients' progress and response to treatment.
- Provide education and support to patients and their families regarding their health conditions and treatment plans.
- Communicate effectively with healthcare providers, caregivers, and other members of the interdisciplinary team.
- Ensure compliance with regulatory standards and company policies and procedures.
- Document all patient interactions, assessments, and interventions accurately and timely.
- Current licensure as a Licensed Vocational Nurse (LVN) in the state of California.
- Minimum of 1-2 years of experience working as an LVN in a healthcare setting, preferably in care management or case management.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Proficiency in computer skills and electronic health record (EHR) systems. Epic experience preferred.
- Commitment to providing patient-centered care and promoting positive patient outcomes.
