We are seeking a highly skilled Ambulatory Case Manager to join our team. As an Ambulatory Case Manager, you will play a vital role in supporting patient care coordination in the primary care ambulatory clinic setting.
You will optimize care coordination with hospital, ED, consulting physicians, and community resources as necessary. This includes collaborating with physicians, providers, and practice caregivers to identify appropriate patients for care management.
Maintaining a working knowledge and relationship with community resources and payer benefits is essential in this role. You will assist in maintaining expertise on benefits, reimbursement, and contract/regulation changes per payer, managed care contract guidelines, Medicare, and Medicaid.
Collaborating with the interprofessional health team across healthcare settings is critical to effectively implementing coordination plans while maintaining fiscal accountability and individual patient advocacy.
Key Responsibilities:
- Optimize care coordination with hospital, ED, consulting physicians, and community resources as necessary.
- Maintain a working knowledge and relationship with community resources and payer benefits.
- Collaborate with the interprofessional health team across healthcare settings.
Requirements
To be successful in this role, you must have a completion of a nursing education program approved by a board of nursing. Additionally, you must have Basic Life Support (BLS) certification within 60 days of hire or transfer, and Registered Nurse (RN) licensure in South Dakota.
PREFERRED QUALIFICATIONS:
Bachelor's degree in Nursing
Commission for Case Management Certification (CCMC)
