**Job Description**

The Case Manager will deliver assistance crucial to enhancing or improving a client's standard of living, emotional health, physical health, spiritual health, and community health.

The Case Manager will strive to remove barriers impeding upon a person's level of self-sufficiency.

Traveling 80-90% of the time with only about 10% of office work, this position is primarily case management for the homeless population in the Coachella Valley.

**Key Responsibilities**
  1. Assess displaced clients and family needs out in the field.
  2. Develop comprehensive care plans for families to become self-sufficient.
  3. Coordinates needed services.
  4. Maintains up-to-date client records.
  5. Provides crisis and/or short-term counseling.
  6. Provides information and referral to community and staff.
  7. Engages in outreach activities.
  8. Ensure compliance with all Department of Housing and Urban Development policies.
