**Job Description**The Case Manager will deliver assistance crucial to enhancing or improving a client's standard of living, emotional health, physical health, spiritual health, and community health.
The Case Manager will strive to remove barriers impeding upon a person's level of self-sufficiency.
Traveling 80-90% of the time with only about 10% of office work, this position is primarily case management for the homeless population in the Coachella Valley.
**Key Responsibilities**- Assess displaced clients and family needs out in the field.
- Develop comprehensive care plans for families to become self-sufficient.
- Coordinates needed services.
- Maintains up-to-date client records.
- Provides crisis and/or short-term counseling.
- Provides information and referral to community and staff.
- Engages in outreach activities.
- Ensure compliance with all Department of Housing and Urban Development policies.
- Comprehensive, low-cost healthcare coverage for employees.
