Key Responsibilities- Lead a multi-disciplinary team of healthcare providers to deliver high-quality patient care
- Implement our proven care model, which focuses on treating the whole patient - physically, mentally, and socially
- Typically see 14 to 16 patients per day, with a patient panel predominantly consisting of geriatrics with Medicare Advantage coverage
