The Nurse Care Coordination Specialist will manage the flow of referrals from various healthcare facilities and providers. This role involves diverse tasks that often necessitate independent judgment and interpretation of suitable actions.

The Nurse Care Coordination Specialist will be part of CenterWell Home Health, providing support to a designated market area while working closely with referral management teams. This position will focus on streamlining the referral and authorization processes.

Your responsibilities will extend to collaborating with other healthcare facilities within the designated market area, ensuring comprehensive support. The facilities you will engage with include:

  • Various Military Treatment Facilities
  • Local Civilian Healthcare Providers
  • Community Health Organizations

Key Responsibilities:

40% Facilitate and resolve referral and authorization processes while fostering collaborative relationships with healthcare associates.
25% Review and support understanding of healthcare benefits and medical necessity requirements.
15% Identify and connect cases to appropriate care management programs and quality assurance initiatives.
10% Participate in meetings and provide insights to enhance operational efficiency.
10% Communicate feedback to management regarding any issues that may require further attention or coordination.

Work Environment: This position requires onsite presence five days a week within a healthcare office setting.

This role description does not encompass all responsibilities and may require additional tasks as assigned by management, aligned with departmental goals and objectives.

Required Qualifications

  • Active RN (Registered Nurse) license in the relevant state(s).
  • Minimum of three years of diverse clinical nursing experience.
  • Strong relationship management capabilities.
  • Proficient in Microsoft Office Suite (Outlook, Word, PowerPoint, Excel).
  • Excellent verbal and written communication skills.
  • U.S. citizenship is a requirement for this position.
  • Ability to obtain government security clearance.

Preferred Qualifications

  • Experience with TRICARE or similar healthcare programs.
  • Three or more years in utilization or case management roles.
  • Professional certification in case management or quality assurance.
  • Experience with healthcare data and informatics.
  • Familiarity with MCG guidelines.
  • Strong organizational skills.
  • Bachelor's degree in Nursing (BSN) is preferred.
  • Prior leadership experience in a supervisory role is advantageous.

Work Environment: Office setting.

Scheduled Weekly Hours: 40 hours per week.

Compensation: The salary range reflects a good faith estimate for full-time employment, which may vary based on location and individual qualifications. This position may also be eligible for a performance-based incentive plan.
