Key Responsibilities

The role of Case Management involves a collaborative approach to assessing, planning, facilitating, and advocating for the necessary services to meet patients' health requirements while promoting effective and economical outcomes. The primary objective is to assist patients in achieving optimal health and navigating available services while respecting their rights to make informed decisions.

Essential Duties Include:

  • Coordinating discharge plans for patients
  • Identifying complex discharge planning requirements
  • Managing internal resources for patient care plans
  • Facilitating transitions to the next level of care (e.g., TCU, Rehabilitation, Long-Term Acute Care)
  • Participating in daily discharge team meetings
  • Engaging professionally with patients, families, physicians, and other healthcare professionals to develop care plans

Qualifications

Required Education:

A nursing degree from an accredited institution is required, with a preference for a baccalaureate degree in nursing. Candidates must hold a valid nursing license in the state of practice.

Required Experience:

A minimum of 2-3 years of clinical experience, primarily in medical/surgical nursing, is essential.

Knowledge, Skills, and Abilities:

Candidates should possess the ability to prioritize tasks, meet deadlines, and maintain productivity standards. Strong interpersonal and communication skills are critical for building collaborative relationships with staff, patients, families, and post-acute care providers. Proactive problem-solving skills and the ability to work independently are also necessary.

Preferred Qualifications:

Experience in case management, discharge planning, or utilization review is preferred.

Compensation:

The pay range for this position is competitive and commensurate with experience.
