Job Summary:
As a Clinical Outcomes Specialist at National Park Medical Center, you will be responsible for managing patient caseloads and ensuring high-quality clinical, quality, and financial outcomes. This role requires strong clinical management skills, discharge planning expertise, and the ability to work closely with physicians, nurses, and other departments.

Key Responsibilities:
Develop and implement effective case management plans to achieve optimal patient outcomes
Collaborate with healthcare teams to ensure seamless care transitions and efficient resource allocation
Conduct thorough assessments and identify opportunities for improvement in patient care
Analyze data to inform clinical decisions and drive quality improvement initiatives

Requirements:
Minimum of 2 years clinical experience
Strong communication and interpersonal skills
Ability to work in a fast-paced environment and prioritize tasks effectively
Detail-oriented and organized with excellent time management skills
