Travel RN Case Manager

Tact Staff is seeking a skilled Travel Nurse RN Case Manager specializing in Acute Care Case Management.
Position Overview:
This role involves overseeing Care Coordination and Care Transitions Planning throughout the acute care patient journey.
The selected candidate will collaborate with physicians, utilization managers, medical social workers, and bedside RNs to ensure patients progress smoothly and transition to the appropriate level of care, thereby preventing unnecessary admissions or readmissions.
The Care Management process emphasizes effective communication and resource coordination across the healthcare continuum.
Key Responsibilities:
- Collaborate with patients, families, and the interdisciplinary team to achieve optimal health outcomes.
- Facilitate access to care and ensure appropriate resource utilization while respecting patients' self-determination.
- Address complex clinical and social situations efficiently, particularly in the Emergency Department, to minimize unnecessary admissions.
Qualifications:
- Active California RN Licensure
- Minimum of 2 years of experience in Case Management
- BLS certification (AHA)
- Experience in Utilization Management, supporting level of care protocols
This position requires a commitment to a 3 x 12 Days shift schedule, including weekend rotation and on-call duties as necessary.
