I. Overview of the Role

This position is classified as Safety-Sensitive. Under the guidance of a Registered Nurse or Physician, the Licensed Practical Nurse plays a crucial role in the healthcare team, contributing to the practice's success through exceptional patient care, effective communication, professionalism, and collaboration. The LPN utilizes the nursing process to assess, plan, implement, and evaluate comprehensive care for designated patient populations and their families.

II. Primary Responsibilities
  • Administers fundamental nursing care to patients by adhering to established protocols and procedures.
  • May provide specialized nursing care pertinent to the practice's focus.
  • Collects patient information, including vital signs, and observes patient behavior, reporting findings as necessary.
  • Prepares and delivers injections, conducts routine tests, treats wounds, and changes dressings.
  • Maintains patient records using standardized medical documentation practices.
  • Administers prescribed medications and monitors patient progress, documenting responses to treatment.
  • Engages in the implementation and assessment of patient care in accordance with practice guidelines and regulatory standards.
  • Documents treatment progress and patient responses meticulously.
  • Educates patients and families regarding health status, maintenance, and management of both acute and chronic conditions.
  • Collaborates with teams to enhance patient care processes and outcomes.
  • Performs additional duties as assigned.
Requirements

III. Qualifications

a. Education / Accreditation / Licensure (Required & Preferred):
  • Graduation from an accredited practical nursing program is mandatory.
  • Possession of a current and valid Licensed Practical Nurse or Licensed Vocational Nurse license from the state licensing authority is required.
  • Basic Life Support (BLS) or Advanced Care Life Support (ACLS) certification is essential.
  • Additional specialty certifications may be required based on practice area.
b. Experience (Required and Preferred):
  • 1-3 years of practical experience in a clinical or hospital environment is preferred.
