Job Summary:
Bloom providers serve as the primary care provider for patients and see patients based on medical necessity. They deliver comprehensive primary care and care management, including add-on services such as on-demand care, wound care, hospice, real-time blood draws, and more. Our providers strive to see patients post-discharge within 72 hours to ensure patient stabilization and understanding of any new medications or instructions.
Responsibilities:
Our Nurse Practitioner will help elderly patients to age in place, deliver comprehensive primary care at home, and assess and address factors such as socioeconomic status, housing stability, and cultural influences to optimize patient outcomes. They will demonstrate exceptional communication and relationship-building skills with care team members, provide data-driven, condition-specific specialty visits to patients as needed, and manage the health outcomes of an empaneled patient population.

Qualifications:
We are seeking a Master's degree holder with licensure in the state of Colorado, with bilingual skills preferred.
