Job Summary

We are seeking a highly skilled Advanced Practice Clinician to join our team at Community Health Center of Southeast Kansas. As a key member of our medical clinical team, you will be responsible for providing primary care services to our patients, including assessing and managing their health needs in a fast-paced walk-in care setting.

Key Responsibilities
  • Develop and manage the medical plan of care for patients, working collaboratively with supervising physicians to ensure high-quality patient outcomes.
  • Provide comprehensive outpatient care, including health examinations, treatment of medical conditions, and follow-up care as needed.
  • Manage acute and chronic illnesses, routine care, and coordinate care plans with patients' behavioral health providers when necessary.
  • Make appropriate referrals to internal and external resources for additional services required.
  • Prescribe medications for patients as indicated, and provide clinical consultation to other providers within the organization.
  • Document patient records accurately, timely, and clearly, adhering to clinic protocols and guidelines.
  • Communicate effectively with patients, guardians, and employees, providing clear written and oral information.
  • Participate in quality improvement activities and contribute to the development of policies and procedures.
Requirements
  • Full licensure to practice medicine in the State of Kansas or Oklahoma, depending on assigned work location.
  • Graduate of an accredited advanced nurse practitioner program.
  • Completion of one-year experience in a clinic setting, preferred.
  • A strong community/public health orientation, preferred.
  • Commitment to serving the underserved.
Work Environment

This position requires the ability to work in a fast-paced walk-in care setting, with frequent interactions with patients, staff, and other healthcare professionals. The employee must be able to sit, stand, and use hands to finger, handle, or feel, with occasional lifting and/or moving up to 10 pounds. Specific vision abilities required include close vision, peripheral vision, depth perception, and ability to adjust focus.
