We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Community Care, Inc. as a Home Health Nurse. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.
Job Summary:The LPN will work under the guidance of a Registered Nurse (RN) to provide direct nursing care to patients, implementing and evaluating care plans, administering medications, and performing various treatments as ordered by physicians. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work independently and as part of a team.
Responsibilities:- Implement and evaluate patient care plans, making suggestions as needed to ensure optimal patient outcomes.
- Administer medications and perform treatments as ordered by physicians, ensuring patient safety and comfort.
- Monitor patient progress, reporting changes and concerns to the RN and/or Case Manager.
- Document patient information accurately and timely, utilizing electronic medical records.
- Communicate effectively with patients, families, and healthcare team members, maintaining confidentiality and adhering to HIPAA regulations.
- Graduate of a state-approved LPN program with course in pharmacology.
- Current LPN license in the State of Pennsylvania.
- Minimum of six (6) months experience in relevant patient care.
- Current CPR certification.
- Reliable transportation and a valid driver's license (when required to travel between assignments).
- Clearances, including Childline, FBI, DIG, Medicheck, and EPLS, as required by regulatory agencies.
