About the Role

We are seeking a highly skilled Medical Social Worker LCSW to join our team at Swedish Medical Center in the south metro Denver area. As a key member of our interdisciplinary team, you will play a vital role in assessing the psychological, social, cultural, and financial needs of our patients.

Job Summary

This position is responsible for coordinating services through an interdisciplinary process, providing a clinical and psychosocial approach to patient care throughout the continuum of care. You will assess patients' needs, develop personalized plans, and collaborate with healthcare teams to ensure seamless transitions.

Key Responsibilities
  • Conduct thorough assessments of patients' psychological, social, cultural, and financial needs
  • Develop and implement individualized plans to address identified needs
  • Collaborate with interdisciplinary teams to coordinate care and facilitate smooth transitions
  • Provide counseling and education to patients and their families
  • Coordinate discharge planning to ensure timely and effective transitions
Requirements

To be successful in this role, you must possess:

  • A Master of Social Work (MSW) from an accredited school of social work
  • Licensed Clinical Social Worker (LCSW) in the state of Colorado with no restrictions
  • Three to five years of recent clinical social work or direct behavioral health experience in an acute care hospital setting
  • Specialty certification in case management preferred
