Job Title: Certified Nursing Assistant

At Affinis Hospice, we are seeking a compassionate and skilled Certified Nursing Assistant to join our team.

Job Summary:

The Certified Nursing Assistant will provide high-quality patient care and support to our patients and their families in a home health setting.

Key Responsibilities:
  • Maintain and promote the independence of patients and families, and assist in maintaining a safe and healthy environment.
  • Participate in Interdisciplinary Group meetings as appropriate.
  • Assist with personal care activities, household tasks, and other essential tasks directly related to patient health.
  • Measure and record vital signs, such as temperature, pulse, respiration, and blood pressure, as ordered.
  • Meet safety needs of patients, including safe use of equipment.
  • Immediately report any changes in patients' conditions or incidents to the Primary Care Nurse/Director of Hospice Services.
  • Accurately document services provided in accordance with Hospice policies and procedures.
  • Participate in staff development programs and assume personal responsibility for education to meet annual requirements.
Requirements:

Completion of a state-recognized and federally compliant training program and/or competency evaluation, and at least six months of experience as a Certified Nursing Assistant.

Compliance with in-service regulations and requirements.
