Job Summary

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner with Matrix Medical Network, you will conduct home risk assessments, medical history reviews, and health education to patients in their homes. You will work collaboratively with our team to provide comprehensive care and improve patient outcomes.

Responsibilities
  • Conduct home risk assessments, medical history reviews, and health education to patients in their homes
  • Work collaboratively with our team to provide comprehensive care and improve patient outcomes
  • Collaborate with physicians, case managers, and other healthcare professionals to provide coordinated care
  • Provide health education and counseling to patients and their families
  • Document patient information and outcomes in a timely and accurate manner
Requirements
  • Master's Degree in Nursing required
  • Current RN and NP licensure in state of practice
  • Board certified by the AANP, ANCC, or AACCN in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
