Nurse Practitioner

Job Summary:

We are seeking a skilled Nurse Practitioner to join our team at Addus HomeCare. As a Healthcare Professional, you will play a vital role in providing high-quality medical care to our patients in the comfort of their own homes.

Key Responsibilities:

• Provide direct patient care as defined in the state Nurse Practice Act.
• Implement plans of care initiated by registered nurses.
• Document patient information accurately and timely.
• Assess and educate patients and their families on self-care techniques and relevant health information.
• Coordinate home health services and communicate patient needs to the registered nurse and Clinical Supervisor.
• Use and prepare medical equipment and supplies effectively and efficiently.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by registered nurses.

Requirements:

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience, preferably in community/home health or medical/surgical settings.
• Current licensure as a licensed practical nurse or licensed vocational nurse in the state.
• Valid CPR certification.
• Reliable transportation, current driver's license, and appropriate automobile insurance.
