Job Summary

We are looking for a skilled and confident Licensed Practical Nurse to join our team. In this role, you will be responsible for planning, implementing, and evaluating resident care in collaboration with a Registered Nurse. You will have the opportunity to make a meaningful difference in the lives of our residents and contribute to the delivery of high-quality care.

Key Responsibilities
  • Provide direct care to residents, including administering medications, performing treatments, and monitoring vital signs.
  • Collaborate with the Registered Nurse to develop and implement individualized care plans.
  • Conduct regular assessments of resident needs and make recommendations for care.
  • Participate in interdisciplinary team meetings to discuss resident care and develop strategies for improvement.
Requirements
  • Active authorization to practice as a Licensed Practical Nurse in Pennsylvania.
  • Ability to work rotating shifts, including weekends and holidays.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a team environment.
