Hartford HealthCare at Home is seeking a compassionate and skilled Hospice Nurse to join our team. As a Hospice Nurse, you will provide high-quality patient care and support to individuals and their families during their end-of-life journey.
Key Responsibilities:
- Conduct comprehensive assessments to establish goals for symptom management and end-of-life care
- Develop and maintain therapeutic relationships with patients and their families
- Collaborate with an interdisciplinary team to address patients' needs holistically
- Utilize state-of-the-art technology to document skilled services provided
Requirements:
- Registered Nurse with a valid CT RN license
- One year of RN experience; Homecare or Hospice experience preferred
- Valid driver's license and reliable transportation required
- Computer proficiency required
