This role involves measuring vital signs, including temperature, respiration, and blood pressure, following the care plan tailored for each patient.

Reviews and acknowledges the assignment form for each specific case.
Delivers Home Health Aide services as delineated in the patient's care plan.

Daily Living Assistance:
The aide assists patients with personal hygiene tasks such as bathing, grooming, and dressing. Additionally, they may prepare meals and support patients during mealtime.

Nutrition and Documentation:
Facilitates meal planning and preparation while providing nutritional support. The aide keeps track of food and liquid intake and offers medication reminders as necessary.

Health Surveillance:

The aide visits patients in their homes or suitable care settings, observing any changes in their health status.

They measure vital signs and report any significant findings to the appropriate supervisor or nursing staff.

Environmental Maintenance:
Conducts routine light housekeeping to ensure the patient's immediate surroundings are clean and safe.

Social Interaction:
Offers companionship and emotional support to clients, particularly those who are homebound and may have limited social interaction.

Ensures the effective and safe delivery of Home Health Aide services.

Maintains precise, comprehensive, and timely clinical records.
Communicates any patient inquiries, concerns, or complaints to the relevant individuals promptly to facilitate corrective actions.
Upholds the confidentiality of patient information.
Implements necessary infection control and safety protocols, reporting any hazards or incidents in a timely and accurate manner.
Utilizes proper body mechanics consistently.
Engages in case conferences as deemed appropriate.
Participates in ongoing professional development and training.

Completes basic training sessions before being assigned to a patient's home.
Engages in a minimum of 12 hours of in-service training annually.
Demonstrates current knowledge of fundamental home health aide practices.
Contributes to the development and revision of clinical and administrative policies and procedures.
Participates in the Quality Assessment and Improvement Program.
Undertakes additional responsibilities as assigned and necessary.
