LPN Care Coordinator at Avamere Rehab of Issaquah

A skilled Licensed Practical Nurse (LPN) is sought after by Avamere Family of Companies to provide top-notch care and leadership in our rehabilitation center. The ideal candidate will have excellent clinical skills, a passion for delivering quality care, and the ability to work collaboratively with our nursing team.
About the Role
This LPN role is a full-time position with shifts ranging from 8 to 12 hours, working days. As an LPN Care Coordinator, you will be responsible for coordinating daily services, programs, and activities to ensure the highest level of care for our residents. You will also prepare and administer medications, review medication cards, and participate in facility surveys.
Responsibilities
- Coordinate with nursing staff and support personnel to plan daily services, programs, and activities
- Prepare and administer medications, reviewing medication cards for completeness and accuracy
- Review resident care plans, ensuring goals, problems, approaches, and revisions align with nursing needs
- Plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures for quality care
- Provide leadership to nursing personnel, making daily rounds to ensure staff provides direct care according to resident care plans and wishes
- Participate in facility surveys by authorized government agencies
- Chart nurses' notes in an informative and descriptive manner, reflecting care provided and resident response to care
Requirements
- Must possess a nursing degree from an accredited college or university
- Hold a current, unencumbered, active license to practice as an LPN in this state
- Knowledgeable of nursing and medical practices, laws, regulations, and guidelines related to nursing care facilities
- Experienced with Electronic Medical Records and computer documentation systems
- Maintain confidentiality of all resident care information according to HIPAA guidelines
