- Coordinate Care: Work with clients, families, and healthcare providers to ensure seamless care coordination and communication.
- Monitor and Evaluate: Continuously monitor clients' progress and adjust care plans as needed to ensure optimal outcomes.
- Document and Communicate: Accurately document client information and communicate effectively with clients, families, and healthcare providers.
- Education and Experience: Current RN licensure and 2 years of nursing experience, preferably in home health or hospice care.
- Certifications and Skills: Current CPR certification and ability to perform all nursing procedures under the direction of a physician.
- Personal Qualities: Compassion, empathy, and excellent communication skills.
- Competitive Pay: $30.00 per hour, with pay may vary by location, level of training, and type of shift.
