  • Coordinate Care: Work with clients, families, and healthcare providers to ensure seamless care coordination and communication.
  • Monitor and Evaluate: Continuously monitor clients' progress and adjust care plans as needed to ensure optimal outcomes.
  • Document and Communicate: Accurately document client information and communicate effectively with clients, families, and healthcare providers.
Requirements:
  • Education and Experience: Current RN licensure and 2 years of nursing experience, preferably in home health or hospice care.
  • Certifications and Skills: Current CPR certification and ability to perform all nursing procedures under the direction of a physician.
  • Personal Qualities: Compassion, empathy, and excellent communication skills.
