Care Coordinator Specialist

The estimated salary for this position is $60,000 - $80,000 per year.
Job DescriptionIn this role, you will be responsible for overseeing the Resident Assessment Instrument (RAI) process, ensuring timely and accurate completion of the Minimum Data Set (MDS), care plans, and other related tasks. You will work closely with the interdisciplinary team to ensure compliance with Federal and State guidelines.
Your primary responsibilities will include:
- Ensuring RAI and documentation meet regulatory and reimbursement standards;
- Training and educating interdisciplinary team members on the RAI process and quality measures;
- Completing initial, quarterly, annual, and special MDS and care plans according to State and Federal guidelines;
- Submitting MDS electronically to the facility's chosen scrubber and iQIES;
- Reporting resident conditions and services provided according to Federal and State guidelines to maximize quality measure ratings and reimbursement;
- Maximizing CMI and educating skilled nursing staff on MDS processes and documentation;
- Partnering with departments for accurate and timely billing related to MDS;
- Reviews medical records and attends interdisciplinary team meetings as needed;
- Develops care pathways and communicates information to residents, family, and healthcare professionals;
- Reviews state validation reports and takes follow-up action;
- Schedules MDS for maximum reimbursement;
- Codes MDS accurately with information from medical records, observation, and interviews;
- Maintains MDS and care conference schedules;
- Completes assigned assessments for MDS completion;
- Reviews hospital records and MD/NP documentation for MDS information;
Required Skills and Qualifications
To be successful in this role, you will need to possess the following skills and qualifications:
- Ability to supervise and direct nursing staff;
- Effective communication, conflict resolution, and problem-solving skills;
- Knowledge of aging process, resident rights, and abuse regulations;
- Determining nursing staff and supplies needed for quality resident care;
- Correct and safe use of nursing/resident equipment;
- Effective oral and written communication;
- Professionalism and business acumen for working with guests, clients, and employees;
- Excellent time management, punctuality, and sense of urgency;
- Cost-effective resource management;
Education and Experience
To apply for this position, you must have a current RN license in New York State and at least one year of experience in skilled nursing facilities, rehabilitation nursing, or other healthcare-related settings. Previous experience with MDS, RAI process, and care plans is highly desirable. Maintaining current RAC-CT certification is also required.
