As an Independent Optometrist at Walmart, you will have the autonomy to establish your own practice within our Sam's Club locations. This unique opportunity allows you to take a leading role in patient care, providing eye and vision care as well as general health and well-being services.

• Establish your own days of practice and hours of operation
• Control your patient schedule and set your fee schedule
• Keep 100% of exam/professional fees
• Option to opt into equipment/instrumentation and administrative support services (if state allows)

• Doctor of Optometry (OD) from an accredited optometry school
• Valid state optometry license
• Ophthalmologist (MD or DO) from an accredited school
• Valid state ophthalmology license
• General Liability Certificate
• Professional Liability Insurance Certificate
• W-9
