Occupational Therapist Home Health Full Time

At CenterWell Home Health, we're dedicated to delivering exceptional, patient-centered care in the comfort of our patients' homes. As a Home Health Occupational Therapist, you'll play a vital role in helping patients regain their independence and improve their quality of life.
Key Responsibilities:- Conduct thorough assessments to identify patients' needs and develop personalized treatment plans.
- Collaborate with healthcare teams to ensure seamless care coordination and effective communication.
- Provide expert occupational therapy services, including therapeutic interventions and education, to promote patients' functional abilities and independence.
- Document patient progress and outcomes accurately and thoroughly, ensuring timely communication with healthcare teams and patients' families.
- Participate in care planning and discharge planning activities to ensure a smooth transition to the next level of care.
- Master's degree in Occupational Therapy from an accredited program.
- Minimum of six months of occupational therapy experience, with a preference for home health experience.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Excellent communication and organizational skills.
- Valid driver's license, auto insurance, and reliable transportation.
