About the Role:
The LPN Team Leader is a pivotal position that combines direct patient care with operational management within our healthcare center. This role is designed for individuals who thrive in a dynamic environment and are passionate about providing exceptional healthcare.
Key Responsibilities:
- Support primary care providers in delivering patient care while overseeing daily center operations to ensure efficiency and quality.
- Enhance patient experience by managing scheduling, minimizing wait times, and addressing operational concerns.
- Assist the Center Clinical Manager in supervising Care Promoters and support staff, contributing to performance evaluations and improvement plans.
- Train and mentor new and existing staff on health education and clinical procedures under the guidance of the Center Clinical Manager.
- Prepare patients for examinations, conduct routine screenings, and assist physicians during patient visits.
- Manage patient communications, including phone messages and prescription requests, ensuring compliance with medical protocols.
- Oversee quality assurance activities within the center, including equipment checks and inventory management.
- Participate in quality improvement initiatives aimed at enhancing patient safety and outcomes.
Qualifications:
- High school diploma or equivalent; graduation from an accredited nursing program is required.
- Active LPN license in the state of employment.
- IV Therapy Certification may be required based on state regulations.
- A minimum of one year of LPN experience is essential; three years of overall clinical experience is preferred.
- Experience with geriatric patients is advantageous.
- Basic Life Support (BLS) certification is required within the first 90 days of employment.

- Strong leadership and communication skills, with the ability to resolve conflicts effectively.
- Excellent organizational skills and the ability to prioritize tasks in a fast-paced environment.
- Commitment to providing outstanding customer service to patients and their families.
