About the Position

We are seeking a qualified Long-Term Care RN to join our team at a skilled nursing facility in Portland, Maine. As a Long-Term Care RN, you will play a vital role in providing patient care and support to residents in a dynamic and fast-paced environment.

Job Duties and Responsibilities

  • Develop and implement individualized care plans for patients
  • Monitor patients' long-term progress and adjust care plans as needed
  • Supervise and coordinate the care of patients, including hands-on care and medication management
  • Collaborate with other healthcare professionals to ensure high-quality patient care

Qualifications and Requirements

  • 1-2 years of recent experience in Long-Term Care or a related field
  • Active license or accreditation in one of the 50 US states
  • Additional requirements and certifications as may be requested by the facility
