We are seeking a compassionate and skilled Home Healthcare Provider to join our team at Abode Care Partners.
Key Responsibilities:- Deliver high-quality, evidence-based medical care to seniors and patients with complex needs in the comfort of their own homes or residential facilities.
- Collaborate with caregivers and healthcare professionals to provide interdisciplinary care and ensure comprehensive patient plans.
- Conduct thorough examinations, diagnose chronic and urgent conditions, and develop treatment plans tailored to each patient's unique needs.
- Maintain accurate and up-to-date patient records, ensuring seamless communication among healthcare providers.
- Achieved state Nurse Practitioner licensure.
- Current National Board Certification for NPs.
- Active state DEA licensure.
- Possession of a Master's degree in Nursing (MSN).
- Minimum 5 years of nursing experience, with at least 2 years as a Nurse Practitioner preferred.
- Exceptional interpersonal and communication skills, both written and verbal.
- Demonstrated proficiency in electronic medical record systems, Microsoft Office applications, and HIPAA/OSHA regulations.
- Licensed driver with valid automobile insurance.
