Job Summary:
The Nurse Practitioner will collaborate with the Interdisciplinary Team to create, review, and revise patient care plans. This role requires providing direct medical care to hospice patients through home visits and advising nursing staff. The Nurse Practitioner will perform face-to-face visits under the supervision of the Medical Director.
Key Responsibilities:- Collaborate with the IDT to develop and implement patient care plans.
- Provide medical care to patients in their place of residence, adhering to the scope of practice.
- Advise Hospice nurses on patient care issues and consult with the Medical Director on medical matters outside of the Nurse Practitioner's scope.
- Provide education and counseling to patients and caregivers on preventative care and medical issues.
- Visit assigned patients to evaluate their continued eligibility for Hospice services, including performing face-to-face encounters.
- Document patient information and services provided in the electronic medical record.
- Maintain expertise in hospice nursing and medical practices through ongoing education.
- Establish and nurture relationships with staff and community referral sources to facilitate program growth.
- South Carolina Nurse Practitioner Certification/SC License is required.
- Prior management experience in long-term care or hospice experience is preferred.
- Computer skills are required.
The Nurse Practitioner will work in office areas, community settings, and long-term care facilities. The role requires frequent travel within the agency's service area and the ability to operate a personal automobile. The Nurse Practitioner will be subject to hostile and emotionally upset patients, family members, and visitors. The role requires working beyond normal working hours and on weekends and holidays when necessary.
