Role Overview

The selected nurse will be responsible for supporting patients with various medical needs, including wound management, medication administration, and other duties as assigned by the healthcare provider. While prior experience in a home care environment is advantageous, it is not mandatory. Additional responsibilities encompass assisting with Activities of Daily Living (ADLs), ensuring patient safety, and addressing personal care requirements. This role involves close collaboration with the Nurse Care Manager to guarantee that clients receive the appropriate care and assistance necessary to uphold their independence and safely remain in their residences.

Qualifications
  • Valid Nursing License in Illinois
  • A minimum of 4 years of nursing experience
  • Proficiency in using mobile applications for clocking in and out, as well as documenting case notes
  • Ability to work effectively as part of a team
  • Capability to operate independently and make informed decisions when necessary
  • Previous experience in home health care is preferred but not essential
