Job Summary

We are seeking a highly skilled and experienced MDS Coordinator to join our team. The successful candidate will be responsible for assigning ICD-10 CM diagnosis codes to residents' medical conditions, managing case loads, and coordinating personalized care plans.

Key Responsibilities

  • Case Management: Assign ICD-10 CM diagnosis codes to residents' medical conditions and manage case loads to ensure accurate and timely assessments.
  • Patient Assessment: Expertly assess nursing home residents' needs and coordinate personalized care plans based on those assessments.
  • Regulatory Compliance: Oversee the patient assessment process, setting assessment schedules and ensuring compliance with regulatory requirements.
  • Care Coordination: Coordinate patient care plans in accordance with regulatory requirements, ensuring that appropriate facility resources are made available to patients.
  • Scheduling: Create schedules for Medicare and Medicaid patients, including start coverage and denial letters.
  • Regulatory Updates: Stay up-to-date on regulatory changes in Medicare coverage and help determine documents needed for Medicaid reimbursement.

Requirements

  • Education: Minimum associate degree, Bachelor's degree preferred.
  • Certifications: Must be a Registered Nurse in good standing, Resident Assessment Coordinator certification.
  • Experience: One year of MDS experience in a skilled nursing/long-term care facility.
  • Skills: Extraordinary attention-to-detail, capable of directing other people and managing schedules, excellent communication skills, and proficiency in MDS 3.0.
