Job Description

At Advanced Home Health Services, we are seeking a skilled Licensed Practical/Vocational Nurse to join our team. As a key member of our home health care team, you will provide direct patient care under the supervision of a registered nurse.

Responsibilities:

  1. Implement the plan of care initiated by the registered nurse.
  2. Provide accurate and timely documentation consistent with the plan of care.
  3. Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  4. Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse and/or Clinical Manager/Clinical Director.
  5. Use equipment and supplies effectively and efficiently.
  6. Participate in personal and professional growth and development.
  7. Perform other duties as assigned by the registered nurse.

Requirements:

  1. Graduate of an accredited practical nurse or vocational nursing program.
  2. Has three years nursing experience and, effective January 1, 1998, Shall have one year prior professional nursing experience in Community/home health or medical/surgical experience.
  3. Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
  4. Complies with accepted professional standards and practice.
  5. Demonstrates good verbal and written communication, and organization skills.
  6. Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
  7. Possesses and maintains current CPR certification.

Skills Required:

  1. Self-directed and able to work with minimal supervision.
  2. Demonstrates excellent observation, problem solving, verbal and written communication skills, nursing skills per competency checklist.
  3. Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills
  4. Pull, push and or lift 15-20 pounds on a daily basis. Pull / push 125-200 pounds on a regular basis. Pulls/push 200 or more pounds on a regular basis
  5. Sits, stands stoops, squats, walks, and moves intermittently during work hours.
  6. Drives in own car to and from patients' home. Work in patient(s) homes as well as agency office.
  7. Ability to cope with the mental and emotional stress of the position.
